Study of hepatic dysfunction in dengue fever

【 摘 要 】

Hepatic involvement is a common feature in dengue fever, which prolongsthe clinicalcourse of thisself-limiting viral infection andconstitutes asignof worseprognosis. Vomitingandabdominal pain in theearly stages pointtowards hepatic dysfunction. Various studieshaveshown that inallthose patients who develop complications like dengue haemorragicfever, dengue shock syndrome,Hepaticfailure,ARDS,RenalfailureandSepticaemia,thelevelsofASTandALT were raisedsignificantly.Theseverity ofhepaticinvolvementcanbeamajorcontributing factor in morbidityandmortalityof such patients with Denguefever. SoASTand ALTcanbeausefulearlymarkertoassessthe severityofthediseasewhichcan thereby lead to earlyrecognition of high risk cases. Methods:Thestudy isanobservationaldescriptivestudy conductedatVydehiInstituteofMedical SciencesandResearchCentre,Bangalore, in the dept of medicine between Jan 2013- Jan 2015 tostudytheprofileofliverinvolvementamongagroupofadultpatientssufferingfrom denguefeverand theassociationofelevatedliverenzymeswithcomplications.Detailedhistory,a complete generalphysicalandsystemicexamination,with relevant investigations were done on hundred patients as per the proforma.. Results:The results were analysed using SPSS-19.A total of 100 patientsofdenguefever were studied,outofwhichseventy patients hadelevatedASTlevelsandseventy threehad elevated ALTlevels.Feverfollowed by headachewerethemostcommonsymptomsat presentation whileVomitingandpainabdomenintheearly stagesuggested hepaticdysfunction.ASTandALTwerestatisticallyhigherinthesepatientsandinthose developing complications like DHF, DSS, hepaticfailure, ARDS,ARFandencephalopathy.Conclusion:Liver injury is universal in adult patients with Dengue fever. Though liver involvementisasymptomatic ina large majority,insomepatientsitleadstoclinical manifestations.Severity ofhepaticinvolvementcanbeamajorcontributing factorin morbidityand mortalityofsuchpatientswith Denguefever. So ASTand ALTcan beauseful early markertoassesstheseverity ofthediseasewhichcanthusleadtoearlyrecognitionof high risk cases.
