【 摘 要 】
Asthmaisrecognizedasaheterogeneousdisorder,althoughinmostpatients,theclinicalmanifestationsareeffectivelymanagedwithestablishedcombinationtherapies.However,5–10%ofasthmaticshavesevereasthma,whichdoesnotrespondtotreatment,andthesepatientsaccountfor>50%ofasthma‐relatedhealthcarecosts.Newinvestigationsintothepathogenesisofglucocorticoidresistanceinsevereasthmaindicatethatpulmonarymacrophagesmayplaycentralrolesinpromotingairwayinflammation,particularlyinasthmathatisresistanttosteroidtherapy.Importantly,factorsthatarelinkedtotheactivationofpulmonarymacrophagesmaycontributetoglucocorticoidresistanceandsevereasthma.Here,wereviewrecentadvancesinunderstandingtherolesofpulmonarymacrophagesinthemechanismsofglucocorticoidresistanceandthepathogenesisofsevereasthma.Wediscusstheroleofmacrophagephenotype,infection,IFN‐γ,LPS,associatedsignalingpathways,TNF‐α,MIF,andothermacrophage‐associatedfactors.Understandingthepathogenesisofsteroid‐resistantsevereasthmawillcontributetotheidentificationofoptimaltherapeuticstrategiesfortheeffectivemanagementofthedisease...
