Sepsisremainstheprimarycauseofdeathfrominfectioninhospitalpatients,despiteimprovementsinantibioticsandintensive‐carepractices.Patientswhosurviveseveresepsiscandisplaysuppressedimmunefunction,oftenmanifestedasanincreasedsusceptibilityto(andmortalityfrom)nosocomialinfections.Notonlyisthereasignificantreductioninthenumberofvariousimmunecellpopulationsduringsepsis,butthereisalsodecreasedfunctionintheremaininglymphocytes.Withintheimmunesystem,CD4Tcellsareimportantplayersintheproperdevelopmentofnumerouscellularandhumoralimmuneresponses.DespitesufficientclinicalevidenceofCD4Tcelllossinsepticpatientsofallages,theimpactofsepsisonCD4Tcellresponsesisnotwellunderstood.RecentfindingssuggestthatCD4Tcellimpairmentisamultiprongedproblemthatresultsfrominitialsepsis‐inducedcellloss.However,thesubsequentlymphopenia‐inducednumericalrecoveryoftheCD4Tcellcompartmentleadstointrinsicalterationsinphenotypeandeffectorfunction,reducedrepertoirediversity,changesinthecompositionofnaiveantigen‐specificCD4Tcellpools,andchangesintherepresentationofdifferentCD4Tcellsubpopulations(e.g.,increasesinTregfrequency).Thisreviewfocusesonsepsis‐inducedalterationswithintheCD4Tcellcompartmentthatinfluencetheabilityoftheimmunesystemtocontrolsecondaryheterologousinfections.TheunderstandingofhowsepsisaffectsCD4Tcellsthroughtheirnumericallossandrecovery,aswellasfunction,isimportantinthedevelopmentoffuturetreatmentsdesignedtorestoreCD4Tcellstotheirpresepsisstate...
