Overnutritionandasedentarylifestylehaveresultedintheexpansionofhumanobesityandassociatedmetaboliccomplications.NonalcoholicfattyliverdiseasehasbecomethemostcommonchronicliverdiseaseinWesterndevelopedcountriesandcanrangefromsimplehepaticsteatosistoacombinationofsteatosis,inflammation,andballooningdegeneration(nonalcoholicsteatohepatitis).Obesityanditsrelatedliverdiseasearebothriskfactorsforhepatocellularcarcinoma,theincidenceofwhichisexpectedtoincreaserapidly.Thepathogenesisofnonalcoholicfattyliverdiseaseanditsprogressiontononalcoholicsteatohepatitisandhepatocellularcarcinomainvolveaderegulatedlipidmetabolismandadisruptionofimmunehomeostasisandtissueintegrityandareassociatedwithastateofchronicinflammation.Macrophagesareimmunecellsessentialformaintenanceoforganfunctionandhomeostasisbutcanalsocontributetotissuedamageandmaintainaproinflammatoryresponse.Theirfunctiondependsontheirorigin,andtissueandcanbeconvertedbasedonlocalenvironmentalcues.Residentlivermacrophages,Kupffercells,whichfunctionassentinels,provideafirstdefenseandareassistedbyinfiltratingmonocytesincasesofhepaticinsult.Untilnow,thecontributionoftissue‐residingandinfiltratingmacrophagestotheonsetandprogressionofnonalcoholicfattyliverdisease,nonalcoholicsteatohepatitis,andhepatocellularcarcinomahasbeenonlypartiallyunraveled.Thisreviewsummarizesthecurrentknowledgeonthecontributionofmacrophagesubsetstoobesity‐drivenfattyliverdiseaseanditscomplicationsandshedslightonstillunexploredareas...
